It is caused by a primary infection with the varicella-zoster virus (VZV). Varicella occurs worldwide and, in the absence of a vaccination program, affects nearly everyone by middle adulthood. VZV is highly contagious and spreads through respiratory droplets or direct contact with the characteristic skin lesions of an infected person. The first clinical symptoms of varicella generally appear after an incubation period of 10–21 days and include fever, malaise, and a characteristic itchy rash.

Chickenpox is generally self-limiting, and the vesicles gradually form scabs, which resolve over a period of 7–10 days. An individual remains contagious until all lesions have formed scabs. The disease is usually mild, but severe complications can arise, including bacterial infections (e.g., cellulitis, pneumonia) and neurological complications (e.g., encephalitis), and these can be fatal. The disease is associated with higher morbidity and mortality in infants and individuals with compromised immune systems.

Chickenpox can be prevented through immunization, and there are several vaccine formulations based on live-attenuated vaccines.. The varicella vaccine is available as a single-antigen vaccine and in combination with the measles, mumps, and rubella vaccine. It is not known how long vaccinated individuals remain protected against varicella. However, live vaccines generally provide long-lasting immunity.

Several studies have shown that people who have been vaccinated against chickenpox have antibodies for at least 10 to 20 years after vaccination. Those who have had chickenpox do not need to be vaccinated against it.

The varicella vaccine is administered in two doses.

A child should receive the first dose between 12 and 18 months of age.
